Fitness Training Sessions

  • 60-minute, structured fitness session led by a licensed Physical Therapist Assistant (PTA)

  • For individuals who have recently discharged from physical therapy but still need guided, progressive exercise

  • Focuses on building strength, endurance, balance, and confidence in a safe, clinically informed way

  • Bridges the gap between formal PT and independent exercise

Valley PM&R Physician Services

Valley PM&R does not accept Medicare, Medicaid, or private insurance. If your commercial insurance offers out-of-network reimbursement, we can verify your benefits and submit a superbill for reimbursement if requested.

Initial Consultation

$350

  • Review of your medical history, PT, OT and SLP notes if indicated,  labs, imaging and other diagnostics prior to your appointment.

  • 60-90 minute in-person visit with the physician

  • If you determine that a monthly membership is the best option for you, then the initial consult fee will cover all membership services for that month. The initial consult will count as 1 of your 2 visits for the first month. 

Monthly Membership

If you will need frequent follow-ups and/or coordination of care with PT, OT, SLP and other members of your care team, this option may be best for you.

$225

3 month minimum

Includes the Following Services:

  • Up to 2 follow-up visits per month

Telemedicine, in person or in conjunction with therapies as appropriate.

  • Weekly Neuro Rehab Collaborative Team Conference (if appropriate)

  • Communication with other members of your healthcare team (PCP, Specialists, other therapists)

  • Preauthorizations related to PM&R/Therapies

  • Portal, text & phone access to physician during office hours

  • Priority Booking

  • Prescription Renewals

  • Additional medical records review

  • Home visit travel fee based on mileage

  • Discounted access to Groups if available

  • Discounted imaging when available

  • Discounted labs when available

  • Discounted prescriptions when available

  • 24/7 access to all labs, imaging, medication history via secure HIPAA-compliant portal

Pay-As-You Go

If you will not need frequent follow-up visits, this may be the most cost-effective option for you

Follow-Up Visit $125

Up to 30 minute visit in person, via telemedicine, or in conjunction with therapies as appropriate.

The Following Services are $200/hour

(Billed in 15-minute increments)

  • Weekly Neuro Rehab Colab Team Conference

  • Communication with other members of your healthcare team (PCP, Specialists, non-Neuro Rehab Collaborative therapists)

  • Preauthorizations related to PM&R/Therapies

  • Portal, text & phone access to physician during office hours

  • Prescription Renewals - Included When Completed During Visit

  • Additional medical records review

  • Home visit travel fee based on travel time

All active patients will have access to the following:

  • Discounted imaging when available

  • Discounted labs when available

  • Discounted prescriptions when available

  • 24/7 access to all labs, imaging, medication history via secure HIPAA-compliant portal
